When in new game plus, on story mode difficulty, if I skill up my character I am being one shot killed or one hit killed each time.
Have you equipped the Purity perk? It's granted when you start a NG+ and, according to its description, dramatically increases damage in both directions. Perhaps it's not tuned properly for story mode.
